Plugin simplerate updated, rates can be assigned to user

Hi,

I just commited updates to plugin simplerate, and some small changes in core, and DB sql:

With this changes, playSMS 1.4.4 master version (beyond beta3) can assign user to rate cards, and each card have rates.

Different user can have different rates based on what cards they are added.
